BOSTON – The Commonwealth of Massachusetts has set up an online COVID Safe Practices Concern Form for residents to report possible concerns about non-compliance with current travel advisory quarantines, the use of face coverings, social gatherings or other public health rules.
Residents are encouraged to fill out the form if they have concerns or information about non-compliance. The report may be filled out anonymously if the user so desires.
The Department of Public Health or a local Board of Health official may conduct a follow up if a user includes their contact information in the form.
Anyone with a concern about a workplace’s compliance with the Commonwealth’s workplace safety standards and protocols may report it to the Department of Labor Standards here.
